Start out with baby steps..meeting in mutual places..allowing her to see how much you have changed..to regain trust..

What steps have you taken to help your addiction?
Are you in a 12 step program? Did you get a sponsor?
This is something that I would be looking at before I took you back
Also how long you been clean and sober?

Stay clean and keep away from other women. It may take a L O N G time, even years to prove yourself.

The only thing you can do is to be there for her now. To be honest with her, and make sure that you follow through with actions, not just your words. The marriage didn’t get ruined in one day, so, it will not be able to be fixed in one day either. Stay clean, stay sober, and take things slow.
